8 WAYS TO TO KNOW IF YOUR PRIMARY CARE DOCTOR IS OFFERING YOU INTEGRATED CARE

  1. During your primary care visit, does anyone ever ask you if you have emotional or mental health problems? (Screening)

  2. Are specialists in mental health available (in-person, via phone or video-conferencing) in the primary care practice? (Team-based care)

  3. Does anyone from the office/clinic call to check on how you are doing after a visit? (Care management)

  4. Are you asked to provide information on a regular basis regarding how you are doing? (Quality improvement)

  5. Does your primary care doctor refer to specialists in mental health if one is not available in the office? (Measurement-based care)

  6. Are you given information regarding how to manage your illness? (Self-management)

  7. If you are referred to a specialist outside the primary care practice, is your health information sent to that specialist? (Tracking)

  8. Are you provided information on local community resources, such as housing, transportation, and other social support services? (Community links).
